If you are using Preview on a Mac, it's very simple. See image below.

If you are using Adobe Acrobat on a PC, it's also easy but has a few more steps.  See video below. 

If you would rather not use a digital ID or signature, you can certainly print it, sign it, and scan it.  Just make sure to scan in all 3 pages of the application into one file.
